Obesity Prevention, Policy & Management (dba Obesity PPM)

Great Falls, VA 22066, US

Obesity PPM is an innovative disease management company improving population health via patient engagement and care coordination interventions addressing obesity, type II diabetes, and myriad related conditions.

We serve healthcare delivery organizations transitioning to value-based reimbursement, medical device and pharmaceutical companies driving commercialization and utilization of evidence-based treatments, as well as health plans, managed care organizations, Accountable Care Organizations (ACOs), and other healthcare payers seeking to make significant health economic impacts in the populations they serve.

Delivered under the brands of our customers, we fully integrate three service areas to design and administer novel disease management programs: engagement center operations with experienced navigators, health coaches, and educators; HIPAA-compliant cloud-based health IT systems with sophisticated mobile applications; and a broadly multi-disciplinary consultancy of thought leaders in all aspects of obesity and its comorbidities.

Our leadership team brings more than 280 years of combined obesity treatment experience to program design, administration, monitoring and evaluation. Our patient, provider, payer and population engagement services transcend organizational boundaries and orchestrate disparate processes. We activate populations and successfully navigate patients through complex, individualized treatment journeys defined by your clinicians' unique care plans, while delivering powerful analytics that advance patient experience, transparency, quality, and cost efficiency.

Patient Engagement & Growth Services

Patient Engagement Growth Services (PEGS) are a unique bundled service consisting of proprietary IT systems, cloud-based contact center services, and consulting services delivered under a long-term outsourcing model. Obesity PPM's PEGS are highly vertically specialized, bringing deep domain knowledge in the obesity space to our customers, and translating that knowledge into the attainment of superior growth objectives.

Obesity PPM is not a traditional healthcare consultancy -- we provide ongoing general management and administration of the outsourced programs we collaboratively design with our customers.

Our customized, scalable, evidence-based services work to reduce the incidence of obesity within specific populations through a variety of clinical and policy strategies. We serve private sector organizations who provide products and services designed to prevent or treat obesity and its co-morbid conditions, and intend to expand into public sector services in the future.
